The first problem is that the large diodes in the alternator will conduct in
reverse, essentially creating a short. There will be an unusually large
amount of arcing as the terminal makes connection, as this is a high-current
path. But in this case, the battery will win. Thousands of amps will flow
that way and the diodes will go short, creating even more destruction.
There will be very obvious smoke, possibly load noises, and even some burnt
wiring.
It's unlikely the car would make it to the "start" phase, but it's possible.
Any other electronic device containing semiconductors that receives power is
going to present a short, and if you are lucky you will just blow a lot of
fuses, but you could also damage the electronics, such as Engine Computers,
Radios, Control Modules, etc. before the fuse melts.
So at the very least you are going to have to replace an alternator and some
fuses.

Don't get me wrong, I didn't hook up the battery backwards.
But I suspect someone may have done this to a 1991 Mi16 I've bought and
I would really like to findout what all may have gone gafooie.
If anybody will admit to making this same mistake I will keep their name
confidential.
