John,
I haven't done it in a while but I seem to remember
that once you've undone a few screws, carefully pop
out the panel and then lift up to clear the ridge on
the top of the door frame.
Once inside, use a hair dryer to heat the moisture
seal and peel it away from the door. You shouldn't
just cut through this and ruin it -- i've seen cars
leak during rain if this is cut.
When pushing out the dent, try not to break the paint
on the inside with anything sharp since it might rust
from the inside out (since water etc drains down
through the inside).
