Twice in the past year or so, my Mi16 has thrown a screw through the distributor cap. Three screws are used to mount the rotor inside it, and, in each case, one of the three screws loosened up and blew a hole in the distributor cap. Thinking it was just some bad gas or something, I drove the car for weeks, not knowing there was a hole in the cap. The car ran rough when cold, but was close to its usual perfection after warming up.
(It's not the timing belt - the car doesn't run at all with a broken belt.)
